Summary
Blue Cross and Blue Shield of Nebraska updated five medical policies effective July 1, 2026, with new prior authorization requirements for compression pumps, limb compression devices for VTE prophylaxis, and ophthalmologic imaging. Several procedures including quantitative EEG and breast imaging techniques are now classified as investigational.
Action Required
Before July 1, 2026: Billing team must update system to require prior authorization for compression pumps (lymphedema/venous ulcers), postsurgical limb compression devices for VTE prophylaxis, and ophthalmologic imaging procedures including CPT 92133, 0198T, and 92132. Update encounter forms to alert providers of new preauth requirements. Educate providers that quantitative EEG for ADHD/autism/cognitive impairment and breast-specific gamma imaging are now investigational and may not be covered.
