Already effective 03/31/2025: Ranibizumab (Cimerli) was withdrawn from the market by the manufacturer on 3/31/25, and current patients on Cimerli may switch to Lucentis or Byooviz without a new prior authorization. Already effective 06/01/2025: Cohere Health's coverage criteria policies applicable to Commercial plans and Medicaid plans were revised for a list of cardiovascular and musculoskeletal procedures, including cardiac ablation, spinal cord stimulators, knee arthroplasty and kyphoplasty.
