Summary

Aetna's Cerebral Perfusion Studies policy (CPB 0663) sets when CT and MRI perfusion studies of the brain are medically necessary. CT perfusion is covered for evaluating acute stroke (less than 24 hours) including hemorrhagic stroke, transient ischemic attacks, follow-up of cerebral ischemia or infarction, planning and evaluating therapy for arterial occlusive disease, identifying cerebral hyperperfusion syndrome after revascularization, and evaluating the vascular status of solid tumors where MRI is degraded by susceptibility artifact. MRI perfusion is covered for diagnosing and following mass lesions such as glioma, and for assessing acute stroke penumbra, hemodynamic significance of cervical or intracranial stenosis, revascularization efficacy and vasospasm. The policy lists many other uses as experimental, investigational or unproven, including CT perfusion for confirmation of brain death, evaluation of head trauma, and evaluation of vasospasm after aneurysmal subarachnoid hemorrhage.

What to do

Action · 3 steps
01Billing team: Before billing CPT 70472 or 70473 for a CT perfusion study, confirm the diagnosis on the claim matches a covered indication in this policy, such as cerebral infarction (ICD-10 I63.00 through I63.9) or Moyamoya disease (I67.5), and that the policy's selection criteria are met.
02Before billing an MRI perfusion study, confirm the documented indication is one the policy covers, such as diagnosis and characterization of mass lesions like glioma or assessment of acute stroke penumbra.
03Check that the indication is not one the policy lists as experimental, investigational or unproven, such as confirmation of brain death, evaluation of head trauma, or prognostic evaluation of cognitive impairment.
