Summary

Aetna's interferon policy covers Actimmune (interferon gamma-1b) for chronic granulomatous disease, mycosis fungoides/Sezary syndrome and severe malignant osteopetrosis, and considers all other indications experimental, investigational, or unproven. Pegasys (peginterferon alfa-2a) and Besremi (ropeginterferon alfa-2b-njft) each have their own list of covered indications, and several Besremi indications are covered only when Pegasys is unavailable. Aetna considers continuation of therapy medically necessary for members who are experiencing benefit, as evidenced by disease stability or disease improvement, and the Biojector 2000 needle-free injection system is considered a medically necessary alternative to needles for members with exacerbating-remitting MS who cannot safely self-inject due to tremors and decreased coordination.

What to do

Action · 10 steps
01Prescribers: Actimmune must be prescribed by or in consultation with an immunologist or a prescriber who specializes in chronic granulomatous disease (CGD), an endocrinologist for severe malignant osteopetrosis, or a hematologist or oncologist for mycosis fungoides/Sezary syndrome.
02Actimmune for other indications is considered experimental, investigational, or unproven.
03Continuation of Actimmune requires that the member is experiencing benefit, shown by disease stability or disease improvement.
04Pegasys: for continuation of therapy for chronic HCV, chronic HBV, or chronic HEV in liver transplant recipients, all members, including new members, must meet all initial selection criteria.
05Treatment of chronic hepatitis B with Pegasys for more than 48 weeks is considered experimental, investigational, or unproven.
06For chronic hepatitis E in liver transplant recipients, the member must be a liver transplant recipient and have had an inadequate response, intolerance, or contraindication to ribavirin.
07Besremi for essential thrombocythemia requires an inadequate response or loss of response to prior cytoreductive therapy (for example anagrelide, hydroxyurea, or Pegasys), or that Pegasys is unavailable.
08Besremi for myelofibrosis, systemic mastocytosis, mycosis fungoides/Sezary syndrome, primary cutaneous CD30+ T-cell lymphoproliferative disorders, and adult T-cell leukemia/lymphoma requires that Pegasys is unavailable.
09Besremi for polycythemia vera is covered without a Pegasys requirement.
10Actimmune diagnosis codes listed as not covered include malignant neoplasm of pancreas (C25.0 to C25.9) and idiopathic pulmonary fibrosis (J84.112).
