Billing team must implement the following changes.
Update billing software to enforce modifier selection logic—when services are reported on the same date, the system must require providers to select the most specific modifier (XE for separate encounter, XP for different practitioner, XS for separate structure, or XU for non-overlapping service) rather than defaulting to Modifier 59 for non-time-based procedures.
Audit all claim templates and standing orders to identify instances where Modifier 59 is currently used for non-time-based services and replace with appropriate XE, XP, XS, or XU modifiers.
Create a provider education document explaining that Modifier 59 is now restricted to time-based services only and provide decision tree guidance for selecting XE, XP, XS, or XU.
Configure system edits to reject claims that use Modifier 59 with non-time-based codes or that include multiple XE/XP/XS/XU modifiers on a single line.
Ensure medical necessity documentation is included with all modifier-appended claims per policy requirements.
Consequences: Claims submitted with Modifier 59 on non-time-based services or with incorrect modifier combinations will be denied by Independence Blue Cross.
Providers should be notified immediately of these changes to allow adequate time for workflow adjustments.
