(2) Create or update prior auth submission checklist requiring: for CPP—GnRH agonist test results or third-generation LH assay labs, bone age assessment documentation, pathologic cause assessment; for gender dysphoria—diagnosis documentation, informed consent, Tanner stage 2+ confirmation, comorbidity status, fertility preservation counseling documentation; for ovarian preservation—premenopausal status confirmation and chemotherapy regimen details; for porphyria—provider experience documentation. (3) Configure system to enforce prescriber specialty requirements: gender dysphoria cases require pediatric endocrinologist, family/internal medicine, or OB-GYN with mental health consultation for members under 18; porphyria cases require porphyria-experienced provider. (4) Add age gating for gender dysphoria (under 18 requires legal permissiveness per TN state law). (5) Train billing staff on continuation of therapy requirements (CPP: female <12 or male <13; others: full criteria re-check). (6) Flag claims for denial if prior auth not obtained before service delivery. Implementation delay until 12/1/26 allows 11+ months for system updates.
