Summary

Effective January 1, 2027, Anthem Blue Cross (Medi-Cal) will end the Community Supports Short-Term Post-Hospitalization Housing (STPHH) service. No new authorizations will be issued after December 1, 2026, and all existing authorizations expire December 31, 2026. Providers must identify affected members, continue services through authorized periods, and coordinate transition planning including connection to Housing Transition Navigation services, ECM care providers, and permanent housing resources.

By December 1, 2026: Billing and clinical teams must (1) identify all current members receiving STPHH services in the billing system; (2) stop processing new authorization requests for STPHH; (3) ensure no new claims are submitted for STPHH after this date. By December 31, 2026: Providers must complete care transition planning for all affected members, including mandatory referrals to CS Housing Transition Navigation service and ECM care providers. Update member communication templates to notify patients of service termination and available alternatives. Document all transition activities in member records. After January 1, 2027: Do not bill or authorize any STPHH services. Claims submitted for services after December 31, 2026 will be denied.
