Summary

UnitedHealthcare Community Plan of Kentucky revised its spinal fusion and decompression medical policy effective November 1, 2026, adding new clinical coverage criteria references to InterQual guidelines, introducing six new clinical definitions, and clarifying that staged/serial procedures for multisite spinal pathology and certain procedures (dynamic stabilization, facet joint replacement, isolated facet joint fusion, vertebral joint implants) are not medically necessary. Billing teams must update their medical necessity determination processes to align with the new InterQual criteria and identify claims that may be subject to denial under the updated non-covered procedures list.

What to do

by Oct 15, 2026 (before effective date) · 5 steps
01Billing team must update prior authorization workflows to reference the InterQual® CP: Procedures guidelines for Decompression +/- Fusion (Cervical, Lumbar, Thoracic), Fusion (Cervical, Lumbar, Thoracic), and Scoliosis/Kyphosis Surgery procedures.
02Configure billing system to automatically flag claims for the following non-covered procedures for denial: Dynamic Stabilization Systems, Facet Joint Replacement, Isolated Facet Joint Fusion (with or without instrumentation), and Vertebral joint implants (e.g., MOTUS).
03Add system rule to deny claims when staged/serial procedures are submitted for multisite spinal pathology when one session can address all sites.
04Providers must be notified that InterQual® Client Defined, CP: Procedures, Decompression +/- Fusion, Lumbar (Custom) - UHG is the fallback tool when standard InterQual criteria cannot determine medical necessity.
05Update internal coding and documentation guides to incorporate the six new definitions: Anticipated Instability Due to Procedure, Instability, Lumbar Spinal Decompression With Interbody Lumbar Spinal Fusion, Meyerding Classification System of Spondylolisthesis, Radiographic Dynamic Instability, and Unstable Grade I Degenerative Spondylolisthesis.
If missed: Failure to implement will result in incorrect authorization decisions and improper claim processing for Kentucky Medicaid members.
