Summary
ForwardHealth has revised the Prior Authorization/Chiropractic Attachment form with new checkboxes replacing text boxes, and added coverage for CPT code 98943 (extraspinal chiropractic manipulative treatment) effective January 1, 2026. The new PA form must be used immediately as submissions with the old form will be rejected.
Action Required
Immediately: Billing team must download and use the new Prior Authorization/Chiropractic Attachment form (F-11029 04/2026) from the ForwardHealth Portal for all chiropractic PA submissions. Old forms will be rejected. Update billing system to include CPT code 98943 for extraspinal chiropractic treatment, limited to once per date of service and following same PA rules as other CMT codes.
