Summary
Effective April 1, 2026, Ambetter Health will no longer cover HCPCS add-on codes G2211 (complexity add-on for established patient relationships) and G2212 (prolonged E/M services). Billing teams must stop submitting these codes for Ambetter claims and use only primary CPT E/M codes (99202–99215) instead.
Action Required
By March 31, 2026: Billing team must update billing software to deny or block submission of HCPCS codes G2211 and G2212 for all Ambetter claims. Update provider encounter templates and billing instructions to remove G2211 and G2212 references. Notify all providers that these add-on codes are no longer reimbursable by Ambetter effective 04/01/2026 and that only primary CPT E/M codes (99202–99215) should be billed. Add compliance check to claims scrubber to catch any G2211/G2212 submissions for Ambetter and reject before transmission. Failure to comply will result in claim denials and potential recovery actions from Ambetter.
