Summary
Effective January 1, 2027, Florida Blue and Truli for Health are eliminating global obstetric bundles and transitioning to itemized billing for maternity care. Prenatal visits, labor management, delivery, and postpartum care will be billed separately using new CPT codes (12 new codes added, 6 revised, 17 removed). Billing teams must update coding workflows, EHR configurations, and documentation practices to reflect E&M codes for prenatal/postpartum care and new labor management codes by the effective date.

Before January 1, 2027: (1) Billing team must update EHR systems and billing software to reflect new maternity CPT code structure—eliminate global delivery codes and configure separate billing for prenatal visits (E&M codes), labor management (new codes), delivery, and postpartum care (E&M codes). (2) Providers must update documentation templates to support itemized billing and include appropriate diagnosis codes identifying prenatal/postpartum status (TH modifier is encouraged but not required). (3) Update internal billing workflows and staff training to reflect transition from bundled to itemized maternity services. (4) Review Florida Blue and Truli updated Obstetrical Services payment policies in their payer portals. (5) Ensure all affected provider contracts are amended per payer guidance. (6) For 2026 antepartum care with 2027 deliveries: use 2026 global codes for antepartum visits in 2026 (CPT 59425 for 4-6 visits, CPT 59426 for 7+ visits); bill any 2027 antepartum or postpartum care using E&M codes. Failure to update systems will result in claim denials or improper reimbursement under new fee schedules effective January 1, 2027.
