Summary
Aetna Clinical Policy Bulletin 0027 considers vasectomy reversal medically necessary for post-vasectomy pain syndrome when the member has failed nonsteroidal anti-inflammatory medications and local nerve blocks or steroid injections. It considers microsurgical denervation of the spermatic cord medically necessary for chronic scrotal content pain when localized scrotal pain has persisted for 3 months or longer, conservative treatments have failed, and the member has a positive response to a spermatic cord block of 50% or greater pain reduction. Endoscopic vasectomy, the Vasclip implantable clip, the Pro-Vas occlusion method, vasal injection, vasal occlusion and epididymectomy for post-vasectomy pain are listed as experimental, investigational or unproven.
