Transforaminal Epidural Injections (CPB 0722, reviewed 2025-10-20)

Effective date
Not stated in the notice
Identified
Aug 16, 2026

Summary

Aetna considers diagnostic selective transforaminal epidural steroid injections (TFESIs) with imaging guidance medically necessary for chronic radiculopathy when the diagnosis remains uncertain after standard evaluation, and considers an initial therapeutic TFESI medically necessary when physical therapy and non-narcotic analgesics have failed or become intolerant and the radicular pain is consistent with radiologic findings. Additional therapeutic TFESIs are medically necessary when the initial injection produced at least two of the following for at least 2 weeks: 50% or greater pain relief, increased level of function or physical activity, or reduced pain medication use and/or additional medical care such as physical therapy or chiropractic care. A maximum of twelve TFESIs per spinal region is considered medically necessary, administered during a maximum of three sessions for an episode of pain lasting six months, and TFESI sessions more frequent than every two weeks are considered not medically necessary.

What to do

Action · 10 steps
01REQUIREMENTS: Billing team: Bill CPT 64479, 64480, 64483 and 64484 for transforaminal epidural injections only when the selection criteria in the policy are met.
02For diagnostic injections, the chart must support radiculopathy as the policy defines it: pain, dysaesthesias or paraesthesias in a specified dermatomal distribution of an involved named spinal root causing significant functional limitations, plus either a detailed neurologic examination within the prior three months showing loss of strength, altered sensation, or diminished, absent or asymmetric reflexes concordant with nerve root compression, or, within the prior 12 months, a concordant radiologist interpretation of MRI or CT showing compression of the involved named spinal nerve roots, or EMG/NCV studies diagnostic of nerve root compression.
03Initial therapeutic injections require that non-invasive measures such as physical therapy and non-narcotic analgesics have failed or become intolerant and that the radicular pain is consistent with radiologic findings.
04Therapeutic injections should be administered as part of a comprehensive pain management program.
05Up to four TFESIs per session are considered medically necessary when the other criteria are met; more than two contiguous vertebral levels or more than four TFESIs in a single session are considered not medically necessary.
06More than three TFESI sessions per episode of pain, per spinal region, per six months, and more than four sessions per spinal region per twelve months, are considered not medically necessary.
07Claims for TFESIs for indications not listed in the medical necessity section, including ultrasound-guided TFESIs and combined pulsed radiofrequency with TFESI for herpes zoster-related pain or lumbar radiculopathy, are considered experimental, investigational, or unproven.
08CPT 64635 and 64636 are listed as not covered for the indications in this policy.
09ICD-10 codes B02.21 through B02.29 and M54.16 are listed as not covered for the combined pulsed radiofrequency and TFESI procedure.
10The policy history lists a next review date of 08/12/2027; confirm current applicability at the source link before billing.
