Action Required
Effective immediately (policy dated 2025-12-02): Billing team must update all billing system rules to deny CPT 22586 for all Aetna plans. Add this code to the non-covered procedure list in the EMR and billing software with denial reason 'Experimental/Investigational - Not Covered.' Configure system to auto-deny any claims submitted with CPT 22586 before they reach Aetna. Notify all surgeons and spine specialists in the practice that AxiaLIF procedures will not be reimbursed by Aetna. Update patient consent forms to inform patients that AxiaLIF is not a covered benefit. Any claims submitted for CPT 22586 will be denied by Aetna; do not appeal these denials as the procedure is classified as not established/unproven.
