Summary

Aetna considers photodynamic therapy with porfimer sodium (Photofrin) medically necessary for esophageal cancer, endobronchial non-small cell lung cancer and inoperable cholangiocarcinoma (as an adjunct to stenting) when the listed criteria are met, and experimental, investigational or unproven when they are not. Photodynamic therapy with topical photosensitizers such as methyl aminolevulinate, topical 5-fluorouracil and aminolevulinic acid is medically necessary for basal cell carcinoma, cutaneous lesions of Bowen's disease, refractory actinic keratoses, low-risk squamous cell carcinoma in situ where surgery or radiation is contraindicated or impractical, and erythroplasia of Queyrat. Photodynamic therapy is considered experimental, investigational or unproven for other cancers and for a long list of non-cancer indications, and the policy applies to commercial medical plans.

What to do

Action · 5 steps
01Billing team: Bill porfimer sodium photodynamic therapy (CPT 96570 and 96571, HCPCS J9600) only for members who meet the esophageal, lung or cholangiocarcinoma criteria in this policy, such as completely obstructing esophageal cancer or microinvasive endobronchial non-small cell lung cancer at an early stage where surgery and radiotherapy are not indicated.
02Services that do not meet these criteria are considered experimental, investigational or unproven.
03Bill topical photodynamic therapy (CPT 96567, 96573 and 96574, HCPCS J7308 and J7345) for the covered skin indications listed in the policy, including basal cell carcinoma, cutaneous lesions of Bowen's disease, refractory actinic keratoses and erythroplasia of Queyrat, and do not bill these codes for the indications the policy lists as not covered.
04Claims should carry the covered ICD-10 diagnosis codes named in the policy, such as C15.3 to C15.9 for esophageal cancer and C22.0 to C22.9 for cholangiocarcinoma.
05The policy lists HCPCS J7309 (methyl aminolevulinate, marked as a discontinued product) among codes not covered for the indications listed in the policy.
