Immediately effective: Billing team and clinical staff must ensure all sterilization consent documentation uses the official DHCS 8649 form in the appropriate threshold language matching patient language preference. Update sterilization procedure protocols to reference the multilingual forms available on the DHCS website (https://mcweb.apps.prd.cammis.medi-cal.ca.gov/) and Medi-Cal Providers Forms page. Verify that only DHCS 8649 consent forms are accepted for Medi-Cal and Family PACT claims; claims submitted with alternative consent forms will be denied.
