Summary
Effective January 1, 2027, Health Net will discontinue three Community Supports services: Assisted Living Facility Transitions, Personal Care and Homemaker Services, and Short-Term Post-Hospitalization Housing. Providers must identify affected members, continue services through December 31, 2026, and coordinate care transitions. This impacts Medi-Cal in 10 California counties.
Action Required
By December 15, 2026: Billing and clinical teams must identify all members currently receiving Assisted Living Facility Transitions, Personal Care and Homemaker Services, or Short-Term Post-Hospitalization Housing. By December 31, 2026: Cease billing for these three Community Supports services and ensure all authorized service periods end by this date. Immediately: Coordinate with Community Supports providers and care management teams to develop transition plans for affected members, documenting alternative resources or services. Update internal tracking systems to flag these service codes as non-billable effective January 1, 2027.
