Summary
Effective January 1, 2027, Community Health Plan of Imperial Valley is terminating three Community Supports services: Assisted Living Facility Transitions, Personal Care and Homemaker Services, and Short-Term Post-Hospitalization Housing. The billing team must identify all affected members currently receiving these services, ensure services end by December 31, 2026, and coordinate care transitions to avoid service gaps.
Action Required
By December 31, 2026: (1) Billing team must identify and audit all active members receiving Assisted Living Facility Transitions, Personal Care and Homemaker Services, or Short-Term Post-Hospitalization Housing services. (2) Verify that all authorizations for these three services end by December 31, 2026 and are not extended past this date in the billing system. (3) Stop all billing and claims submission for these services effective January 1, 2027. (4) Coordinate with care management and providers to ensure transition plans are documented for affected members before service termination. (5) Update internal tracking systems and member communication protocols to reflect service discontinuation. Failure to stop billing these services after December 31, 2026 will result in claim denials.
