Billing teams must ensure claims are submitted only for covered scenarios: allergen-specific IgE testing (up to 20 allergens/year based on clinical history), total IgE for moderate-to-severe asthma or ABPA symptoms, and annual re-testing for food allergen resolution in pediatric patients. Claims for routine re-testing, same-day testing with skin prick tests, and excluded methodologies will be denied.

By August 1, 2026: Billing team must implement coverage edits in billing software to enforce Healthfirst's allergen testing policy. (1) ACCEPT claims only when: specific IgE testing is limited to ≤20 allergens per year with documented clinical history/physical exam/environmental basis; total IgE testing is for moderate-to-severe asthma OR ABPA symptoms; or annual re-testing for same food allergens in children/adolescents with prior positive results. (2) DENY claims for: routine re-testing to same allergens (except pediatric food allergen monitoring); specific IgE testing performed same day as skin prick testing; ALCAT testing; basophil activation flow cytometry (BAT); IgG/IgA/IgM/IgD in-vitro testing; bead-based epitope assays (e.g., VeriMAP Peanut Dx); or qualitative multi-allergen screens without specific allergen identification. (3) Update encounter forms and provider bulletins to clarify that skin testing is contraindicated for patients with dermatographism, urticaria, uncontrolled asthma, recent anaphylaxis, or on beta-blockers/ACE inhibitors, necessitating in-vitro testing instead. (4) Train front-desk and coding staff on new denial reasons. Consequences: Claims submitted outside these criteria will be denied by Healthfirst, resulting in lost revenue and rework.
