Summary
This policy establishes a new admission processing workflow for residents transitioning from conventional Supportive Living Program (SLP) units to Dementia Care Units (DCU) within the same facility. Billing teams must enter a discharge in MEDI for the conventional SLP with a transfer destination code, then submit a new admission Transition Audit Number (TAN) for the DCU with the same date to prevent service gaps and claim denials.

Effective immediately (July 31, 2026): Billing and admissions teams must implement a two-step process for all SLP-to-DCU transitions at the same facility. Step 1: Enter discharge in MEDI for the conventional SLP admission segment using the transition date as discharge date and code destination as 'transfer to new facility.' Step 2: Submit new admission TAN in MEDI for the DCU admission with identical admission date (no gap). For example, if a resident transitions on 06/02/2026, discharge the conventional SLP segment on 06/02/2026 and submit DCU admission TAN requesting 06/02/2026 start date. Train all admissions coordinators and billing staff on this dual-entry requirement. Failure to use matching discharge/admission dates will result in service gaps, claim denials, and billing errors.
