Summary
Illinois hospitals must submit Average Commercial Rate (ACR) data for their five highest-volume commercial payers to demonstrate compliance with federal Medicaid managed care payment limits. This data collection is required to maintain directed payment eligibility for calendar year 2027. Data submission is mandatory by July 31, 2026, through a secure portal managed by Milliman.
Action Required
By July 31, 2026: Hospital CFO or designated billing administrator must submit ACR data through Milliman's secure portal for the five highest-volume commercial payers. Billing team should: (1) Identify and compile paid claims data from the five largest commercial payers; (2) Ensure data is audit-ready and available for CMS verification; (3) Exclude Medicare, Medicare Advantage, Medicaid MCOs, workers' compensation, and non-market-condition payers from submission; (4) Use the HFS-provided template for data formatting; (5) Register CFO email with Milliman portal or delegate access to authorized billing staff. Attend one of the informational webinars (July 7 at 9:00 AM or July 9 at 2:00 PM) for submission guidance.
