Summary
Humana Medicare Advantage now requires prior authorization for SGLT2 inhibitor medications (Steglatro, Segluromet, Invokana, Invokamet, Invokamet XR) for Type 2 diabetes patients. Patients must have tried or have contraindications to both empagliflozin AND dapagliflozin before these medications will be approved.
Action Required
Before January 1, 2026: Providers must obtain prior authorization for SGLT2 inhibitors (Steglatro, Segluromet, Invokana, Invokamet, Invokamet XR) for Humana Medicare Advantage patients. Document that patients have Type 2 diabetes and have tried or have contraindications to both empagliflozin AND dapagliflozin. Visit www.humana.com/PAL for preauthorization requirements. Claims will be denied without proper prior authorization.
