Summary

Humana Medicare Advantage now requires prior authorization for DPP-IV/SGLT2 combination diabetes medications (Steglujan, Qtern, and generic dapagliflozin-saxagliptin) effective January 1, 2026. Patients must have tried individual DPP-4 or SGLT2 inhibitors first, or have contraindications to them.

Providers must document previous treatment with or contraindications to individual DPP-4 inhibitors (sitagliptin, linagliptin) or SGLT2 inhibitors (empagliflozin, dapagliflozin) before prescribing combination products. Update EMR templates to include diabetes medication history documentation.
