Action Required
Immediately: Hospitals should bill LARC devices separately on CMS 1500 forms using appropriate HCPCS codes (J7297, J7298, J7300, J7301, J7307) for separate reimbursement outside DRG payments. Behavioral Health Residential Facilities (Type B8) must stop billing H0031 and H2019 codes as these are included in the H0018 per diem rate. Update paper claim processes to use Lucinda Console font size 10, ensure ICD-10 codes are used, and verify proper field alignment to prevent claim returns.
