Action Required
Immediately: Billing team must update system to expect error code 5521 (not 5687) when V2020 vision frame limits are exceeded. Remove prior authorization requirements for V2750 anti-reflective coating claims for patients under age 21. Prior auth still required for V2750 when service limits exceeded for patients 21+. Claims will not be automatically reprocessed.
