Summary
Alaska Medicaid now requires prior authorization for high index lenses using specific HCPCS codes V2782 or V2783, replacing the previous miscellaneous code V2799. High index lenses are only covered when correction in either eye is +/- 6.75 diopters or greater.
Action Required
Immediately: Stop using HCPCS code V2799 for high index lens authorization requests. Use only V2782 or V2783 with RT/LT modifiers on Vision Service Authorization Request form. Submit completed forms to Conduent SA unit at 907.644.8131. Update billing system to require prior authorization for high index lenses when prescription is +/- 6.75 diopters or greater.
