Summary
This policy establishes that numerous emerging medical technologies and procedures using Category III CPT codes (0075T-0505T) are considered investigational and not covered by insurance. Providers may bill patients directly for these services but must inform patients they will be financially responsible for costs.
Action Required
Before January 1, 2026: Billing team must update system to flag all listed Category III CPT codes (0075T through 0505T) as investigational/non-covered. Front desk staff must implement patient notification process to inform patients before rendering these services that they will be financially responsible for costs. Update encounter forms and EMR templates to include patient consent documentation for investigational procedures.
