Immediately: Primary care providers in Maryland billing CoCM services must use specific CPT codes (99492, 99493, 99494) with updated reimbursement rates. FQHCs and RHCs must use G0512 code only. Before providing CoCM services, contact the behavioral health ASO at 1-800-888-1965 to verify patient's existing behavioral health services. Update billing system with new rates: 99492 ($161.28), 99493 ($128.88), 99494 ($66.60), G0512 ($145.08).
