Ensure claims use appropriate primary diagnosis codes Z33.1 (pregnancy) or Z39.2 (postpartum) and encourage reporting social determinants codes Z55-Z65 when relevant. Maximum 6 prenatal/postpartum visits and 1 labor/delivery visit per pregnancy without prior authorization.
