01Respiratory pathogen panel (87631) may be considered medically necessary only when the multiplex PCR panel is limited to up to five respiratory pathogens, the patient displays signs and symptoms of a respiratory tract infection shown by a compatible clinical syndrome (elevated temperature, dyspnea, tachypnea or tachycardia), and the results will be used to guide or alter management.
02Gastrointestinal pathogen panel (87505) may be considered medically necessary as part of an evaluation for diarrhea lasting more than seven days with fever, bloody or mucoid stools, severe abdominal cramping or tenderness, or signs of sepsis, or for suspected enteric fever in an individual with recent travel to an endemic region or who has consumed foods prepared by people with recent endemic exposure.
03Molecular syndromic panel testing is not medically necessary if the patient has been previously tested by molecular diagnostic methods for the same pathogens within 14 days for the same clinical indication.
04Documentation must show why an evaluation for more than one pathogen by molecular testing is necessary for patient management.
