Summary
Epidural steroid injections may be considered medically necessary as treatment of cervical or lumbar radiculopathy. Reimbursement is limited to a series of three injections in a calendar year at any one level under any and all circumstances. 01Billing team: Limit epidural steroid injection billing to a series of three injections in a calendar year at any one level.
02Do not bill the drug injected separately, because separate billing for the drug is not allowed and it is included in the administration of the injection.
03If there is no documented pain relief after two injections, no further injection at that level is considered medically necessary.
04If the first two injections were done without fluoroscopic imaging and did not provide documented pain relief, the third injection must be done with fluoroscopic guidance.
05When a transforaminal technique is used, more than one epidural injection in the same block session does not count as more than one injection in the series, and additional epidural injections on the same day are reimbursed decrementally as additional procedures.
06Separate reimbursement is not provided for monitored anesthesia care during these approved procedures.
