Summary
This policy confirms that nutrient/nutritional panel testing (including NutrEval FMV, NutrEval Plasma, and SpectraCell micronutrient tests) remains considered investigational for all indications. Claims using the specified CPT codes for these comprehensive nutritional panels will be denied as not medically necessary.
Action Required
Immediately: Billing team must flag CPT codes 82128, 82136, 82746, 83735, 83785, 84590, 84630, and 84999 as investigational in billing system when used for comprehensive nutritional panel testing. Update encounter forms to warn providers these tests are not covered for nutritional deficiency screening in mood disorders, fibromyalgia, unexplained fatigue, or wellness optimization. Claims will be denied as investigational.
