01All MH (type 33) and SUD (type 03) Oregon Medicaid providers must establish process to monitor and respond to OHA licensing notices sent 60 and 30 days before license expiration.
02Billing team must fax renewed licenses with OHA notice to 503-947-1177 immediately upon receipt.
