01Register for Comagine Health Provider Portal access by emailing completed Provider Portal Registration Packet to ORBHSupport@comagine.org or faxing to 877-575-8309.
02By June 30, 2026: Complete required CHPP training available on Comagine Health website.
03By July 1, 2026: Begin submitting all 1915(i) HCBS residential placement referrals through CHPP portal only.
04Note: This does not replace prior authorization requests through MMIS.
