01DME providers must submit original $50,000 surety bond naming 'State of New Jersey, Department of Human Services' as obligee, complete HCFA-1513 ownership disclosure form, and complete FD-394 Additional Surety Bond Information form.
02Send all documents by certified mail to UNISYS Provider Enrollment Unit.
03Bond must be renewed annually.
If missed: Failure to comply will result in claim denials and potential recovery of past payments.
