Summary
New Jersey Medicaid requires all DME providers to post a $50,000 surety bond and complete ownership disclosure forms by January 1, 1998. Claims for DME services will be denied if providers are not bonded.
Action Required
By January 1, 1998: DME providers must submit original $50,000 surety bond naming 'State of New Jersey, Department of Human Services' as obligee, complete HCFA-1513 ownership disclosure form, and complete FD-394 Additional Surety Bond Information form. Send all documents by certified mail to UNISYS Provider Enrollment Unit. Bond must be renewed annually. Failure to comply will result in claim denials and potential recovery of past payments.
