Genetic Testing for Cardiac Ion Channelopathies
Genetic testing for long QT syndrome, CPVT, Brugada syndrome and short QT syndrome may be considered medically necessary only in the situations the policy lists. For symptomatic individuals, testing may be considered when a definitive diagnosis cannot be made without it, and for long QT syndrome this includes individuals with a Schwartz score below 4 but a moderate-to-high pretest probability. For asymptomatic individuals, testing may be considered only when a close relative has a known variant, or, for long QT syndrome and CPVT, when a close relative was diagnosed clinically and genetic status is unavailable, and all other testing is considered investigational.
