Starting January 1, 2027, Sentara Health Plans will require prior authorization for a long list of codes on Virginia commercial plans, including self-funded (ASO) plans, and for code 88380 on commercial, Medicaid and Medicare products, plus code 22860 on Medicare products. Sentara will also stop covering a separate list of codes on Virginia commercial and self-funded plans.

Effective January 1, 2027, G0545 and the listed counseling, screening and vision codes are not separately reimbursable with an E/M or preventive medicine service for Medicare Advantage and CompleteCare (MAP) members, and CPT 99415 through 99418 are non-covered for those plans. Claims with G2211, G2212 or G0545 billed with an E/M service appended with modifier 25 will be denied, and G2211 is not separately reimbursable for Healthfirst Medicaid members age 21 and older.
Already effective 08/01/2026: Blue Cross VT's Medical Nutrition Therapy and Nutritional Counseling policy (10.01.VT205) considers these services medically necessary when a qualified healthcare professional provides them within the scope of their license and the Documentation Guidelines have been met. Services are considered not medically necessary when they are provided and the Documentation Guidelines have not been met. The plan will consider payment for up to one hour per session, and there is no limit on the number of sessions per member per plan year.

Aetna considers counseling of unproven value for asthma, attention-deficit hyperactivity disorder, and chronic fatigue syndrome, and considers the NutrEval test experimental, investigational, or unproven.
Aetna considers hospitalization for initiation of a ketogenic diet medically necessary for intractable seizures, including Doose syndrome and Lennox-Gastaut syndrome, when all listed selection criteria are met, and for persons with glucose transporter protein type 1 deficiency or pyruvate dehydrogenase complex deficiency. Aetna considers hospitalization for initiation of a ketogenic diet of unproven value for all other indications, including adult super-refractory status epilepticus, Alzheimer's disease, autism spectrum disorder, cancer, migraine prevention, multiple sclerosis, Parkinson's disease, schizophrenia and type 2 diabetes. Hospitalization for initiation of the Atkins diet for intractable seizures, mitochondrial diseases, or other indications is considered experimental, investigational, or unproven.
Aetna considers professional services for pediatric intensive feeding programs medically necessary for infants and children with complex feeding and swallowing disorders when an interdisciplinary team provides the care and every listed condition is met. Those conditions include behavior problems interfering with feeding, a documented diagnosis-specific treatment plan with estimated length of treatment, medical causes of failure to thrive treated without resolving the feeding problem, physician coordination and oversight, one of the listed feeding problems, and no response to initial treatment by a single discipline over a 2-month period. Other indications, such as childhood obesity and Prader-Willi syndrome, are considered experimental, investigational, or unproven, and electrical stimulation for swallowing or feeding disorders is also considered experimental, investigational, or unproven.
