Skilled Home Health Care Nursing Services (CPB 0201, reviewed 2026-04-22)
Aetna's Clinical Policy Bulletin 0201 covers skilled home health nursing services, which must be performed by an RN or LPN on an intermittent or hourly basis in the member's home. Aetna considers these services medically necessary when the member is homebound because of illness or injury, the services are ordered by a physician, physician assistant, or nurse practitioner and are directly related to an active treatment plan of care, and they are provided in lieu of hospitalization, skilled nursing facility confinement, or outpatient services outside the home, and are not primarily for comfort or convenience or custodial in nature. Continued care requires a documented need for ongoing visits in which a specific treatment or participant or caregiver education is provided, a reasonable expectation that the member's condition will improve with ongoing nursing care, and documentation of reasonable efforts to transition necessary care to the member or caregivers.
