Medical Policy and Coding Updates - May 2020
Lipectomy or liposuction for lymphedema or lipedema changed from investigational to medically necessary when criteria are met, including significant physical impairment, no response to at least 3 consecutive months of conservative treatment, and continued use of compression garments after surgery. Pharmacy criteria added Valtoco for epilepsy in patients ages 6 and older, required that patients have tried and failed at least one generic anti-seizure medication for Epidiolex, and added Reblozyl for anemia in adults ages 18 and older with beta thalassemia. Coding updates require medical necessity and prior authorization review for CPT 55874 on and after May 17, 2020, removed that review for HCPCS L8692, and require review for investigative status for Q4205, Q4206, Q4208 through Q4219 and Q4221.
