Change in denial process for lack of prior authorization
Point32Health has implemented a cascading denial policy: when a primary service is denied for lack of prior authorization, untimely authorization, or failure to meet medical necessity criteria, ALL related ancillary services, supporting services, procedures, and medications billed on the same or separate claims will also be denied. This policy is effective immediately for most Point32Health products (July 1, 2026) and will take effect September 1, 2026 for Harvard Pilgrim Health Care Commercial, with specific exceptions for radiology professional components, anesthesia, ambulance, pathology professional components, and pre/post services outside the date of service.
