Upper Limb Prostheses (CPB 0399, reviewed 2026-07-16)
Aetna covers orthoses and prostheses when a qualified practitioner prescribes them, the device is provided within six months of the prescription, and a licensed or certified orthotist or prosthetist provides the services. Myoelectric upper limb and hand prostheses are covered for traumatic amputation or congenital absence at the wrist or above when the member has adequate cognitive and neurologic ability, the remaining arm musculature meets the minimum microvolt threshold, a standard body-powered prosthesis cannot be used or is insufficient for activities of daily living, and no comorbidity such as neuromuscular disease interferes with the prosthesis. The policy allows two test sockets per individual prosthesis, no more than 2 socket inserts per prosthesis, and no more than two replacement liners per prosthesis in 12 months, and it lists duplicate or upgraded prostheses, prostheses primarily for cosmesis, prosthetic donning sleeves, and water prostheses as not medically necessary.
