Definitive Lower Limb Prostheses
Medical Policy DME18, Definitive Lower Limb Prostheses, is effective November 1, 2026. A definitive (permanent) lower limb prosthesis may be considered medically necessary when the individual is motivated to ambulate with it, the residual limb has matured, the individual has achieved a defined functional state (K0 to K4) and is cognitively capable of using it to ambulate effectively, and the individual had an in-person medical evaluation with a comprehensive functional assessment using a validated assessment tool or equivalent standardized measures. Prior authorization is required only for definitive prostheses, not preparatory ones. Replacement may be considered medically necessary for a change in the individual's physiological condition, an irreparable change in the device or a part of it, or repairs costing more than 60 percent of the cost of a replacement. Replacement sockets (L5700 to L5703) also require documented functional or physiological need. Replacements with upgraded features that exceed the individual's functional needs are considered not medically necessary.
