Lower limb prostheses are medically appropriate by functional level, microprocessor-controlled knees and ankle-foot devices are medically appropriate when listed criteria are met, and osseointegration, myoelectric hands with individual digit control and enhanced dexterity upper limb devices are investigational. Back-up devices, prosthetics used for leisure or sporting activities, devices used primarily for cosmetic purposes, and replacements of properly functioning devices are not medically necessary.
Medical Policy DME81, Powered and Microprocessor-Controlled Knee and Ankle-Foot Prostheses and Microprocessor-Controlled Knee-Ankle-Foot Orthoses, is effective April 1, 2026. A microprocessor-controlled knee may be considered medically necessary in amputees when the individual shows a need for ambulation at variable rates or over long distances, or job tasks or functional needs that do not permit full focus on knee control and stability. The individual must also have adequate cardiovascular and pulmonary reserve, adequate stride strength and balance, a Medicare Functional Level of K2 (only when stated conditions are met), K3, or K4, adequate cognitive ability, an amputation from hemipelvectomy through knee disarticulation level (including bilateral), a stable or absent wound, and adequate socket fitting. A microprocessor-controlled knee is considered not medically necessary for Medicare Functional Levels K0 and K1, for limited community ambulators at K2 without the cardiovascular reserve, strength, and balance to improve stability, when the primary benefit is leisure or recreational activity, when the individual cannot tolerate the weight of the prosthesis, for significant hip flexion contracture over 20 degrees, or when the individual falls outside the manufacturer's recommended weight or height guidelines. Replacement of a microprocessor-controlled knee may be considered medically necessary when it is malfunctioning, cannot be repaired, and is no longer under warranty, or when the current prosthetic can no longer meet the individual's functional needs due to a significant change in physiological condition. Powered knee or powered ankle-foot prostheses, microprocessor-controlled ankle-foot prostheses, and microprocessor-controlled knee-ankle-foot orthoses are considered investigational.
Independence Blue Cross reissued policy 11.14.21l on microprocessor-controlled prosthetic knees with a new effective date of 10/01/2025. A new HCPCS code L5827 (electromechanical swing and stance phase control knee) has been added to the experimental/investigational section, meaning this device is NOT covered. Billing teams must ensure claims for L5827 are denied or reviewed for medical necessity prior to claim submission.
Independence Blue Cross reissued Policy 11.14.21l on microprocessor-controlled prosthetic knees for lower-extremity amputees, effective 10/01/2025. A new HCPCS code (L5827) has been added to the experimental/investigation section, meaning this code is not covered. The medically necessary criteria and covered codes (L5615, L5856, L5857) remain unchanged.
Aetna considers the eVAC, the Harmony Vacuum Management System (Vacuum Assisted Socket System, VASS), and the LimbLogic VS Prosthetic Vacuum Suspension System medically necessary for lower limb prostheses when the prosthesis criteria are met and at least one of these applies: excessive pistoning that cannot be resolved by suspension adjustments, excessive residual limb hyperemia or excessive skin hyperhidrosis from prior socket use, or multiple falls in below-knee (trans-tibial) amputees. The use of these systems for multiple falls in above-knee (trans-femoral) amputees is considered experimental, investigational, or unproven. Magnetic panels used to enlarge a trans-tibial prosthetic socket to facilitate limb fluid volume stabilization are considered experimental because the evidence is insufficient.
Aetna's Clinical Policy Bulletin 0578 sets coverage rules for lower limb prostheses and their accessories. An orthosis or prosthesis is medically necessary when a qualified practitioner prescribes it, it will significantly improve or restore mobility related activities of daily living, and it is provided within six months of the prescription by a duly licensed or certified orthotist or prosthetist. Each foot, knee, ankle and hip component is covered at a minimum functional level from K0 to K4 set by the policy. Microprocessor-controlled knees, ankles and feet can only be billed with L5999 when PDAC verification is in place, and the Otto Bock Genium system is stated to be not covered because it is not PDAC verified.
Aetna clinical policy bulletin 0630 covers vacuum and magnetic prosthetic socket systems. It considers the eVAC, the Harmony Vacuum Management System (VASS), and the LimbLogic VS Prosthetic Vacuum Suspension System medically necessary for use with lower limb prostheses when the prosthetic medical necessity criteria are met plus one listed criterion. It considers the use of magnetic panels to enlarge a trans-tibial prosthetic socket experimental, investigational, or unproven.
A microprocessor-controlled knee may be considered medically necessary for individuals with transfemoral amputation who demonstrate a need for long distance ambulation at variable rates or regular ambulation on uneven terrain or stairs, who have adequate cardiovascular and pulmonary reserve for faster than normal walking, and who have adequate cognitive ability to master use and care requirements. A microprocessor-controlled knee is considered investigational for individuals who do not meet these criteria, and a powered knee is considered investigational. A microprocessor-controlled or powered ankle-foot is considered investigational, and HCPCS code L2221 for a microprocessor-controlled ankle system orthosis addition was listed as investigational on 04/01/2026.
Independence Blue Cross reissued Medical Policy 11.14.21l on 10/28/2026, effective the same date, reaffirming coverage criteria for microprocessor-controlled prosthetic knees for lower-extremity amputees at functional levels 3–4. The policy maintains existing medically necessary, not medically necessary, and experimental/investigational classifications; no substantive coverage changes were made, only a routine policy review and reissuance.
