State and School Employees' Health Insurance Plan - Hemophilia Therapies
Coverage for hemophilia therapy for immediate treatment may be considered medically necessary when all criteria are met: the individual has hemophilia A, hemophilia B, factor X deficiency or von Willebrand disease, is currently bleeding, is out of medication and needs immediate use, and has no FDA labeled contraindication to the requested agent. Esperoct, Hympavzi, Jivi, Qfitlia (fitusiran) and Roctavian are considered not medically necessary because other alternatives are covered by the plan for the treatment of Hemophilia A and Hemophilia B. The policy history dates the update that added Qfitlia to that list to 11/01/2025.
