Deferoxamine mesylate is considered medically necessary for transfusional iron overload in members with chronic anemia when the pretreatment serum ferritin is greater than 1000 mcg/L, for aluminum toxicity in members undergoing dialysis, and for hereditary hemochromatosis when phlebotomy is not an option or the member had an unsatisfactory response to phlebotomy, and it must be prescribed by or in consultation with a hematologist or nephrologist. Other chelation therapy is considered medically necessary for conditions including aceruloplasminemia, biliary cirrhosis, Cooley's anemia, cystinuria, Diamond-Blackfan anemia, sickle cell anemia, Wilson's disease, secondary hemochromatosis, and heavy metal toxicity when symptoms are suggestive and toxicity is confirmed by laboratory testing. Chelation therapy for cancer, cardiovascular disease, neurodegenerative diseases, autism, ADHD, and mercury toxicity from dental amalgam fillings is considered experimental, investigational, or unproven.
