Endovenous laser ablation, radiofrequency ablation, polidocanol 1% microfoam (Varithena) and cyanoacrylate adhesive ablation (VenaSeal) are considered medically appropriate for symptomatic varicose veins or venous insufficiency when all of these are met: saphenous reflux of at least 500 milliseconds documented by duplex ultrasound, CEAP class C2 or greater, and a documented indication of ulceration from venous stasis, recurrent superficial thrombophlebitis, hemorrhage from a ruptured superficial varicosity, or persistent symptoms that significantly interfere with activities of daily living after compression therapy and activity modification for at least 3 months have not improved them. Phlebectomy, sclerotherapy and transilluminated powered phlebectomy (TPP/TIPP) are considered medically appropriate for symptomatic varicose tributaries when performed concurrently with or following prior treatment of the saphenous veins. Sclerotherapy and transcutaneous laser ablation of small diameter veins (spider veins or telangiectasia), and any treatment of asymptomatic varicose veins, are considered not medically necessary, while balloon catheter-assisted sclerotherapy, cryoablation, coil embolization, mechanochemical or endomechanical ablation (e.g., ClariVein), photothermal sclerosis (intense pulsed light), and sclerotherapy techniques other than microfoam for great, small or accessory saphenous veins are considered investigational.
Missouri MO HealthNet (Medicaid fee-for-service) revised its compression hose coverage for durable medical equipment. Effective June 1, 2026, adults aged 21 and older are covered for listed compression garments as purchase-only items only with a qualifying lymphedema diagnosis, and most listed garments move from manual pricing to set maximum allowable amounts. The bulletin was posted October 26, 2026, after its June 1 effective date, so the changes already apply to dates of service from June 1, 2026 onward.
Aetna considers electrical stimulation medically necessary durable medical equipment for arterial ulcers, diabetic ulcers, Stage III or IV pressure ulcers, and venous stasis ulcers when it is used as adjunctive therapy after no measurable signs of healing for at least 30 days of conventional wound treatment. Courses exceeding 1 hour per day are not considered medically necessary, continued stimulation is not medically necessary if measurable healing is not shown within a 4-week treatment period or once the wound bed is 100% epithelialized, and electrical stimulation for chronic ulcers in the home setting is considered not medically appropriate. Aetna considers electrical stimulation experimental, investigational, or unproven for uses including prevention of ulcers and pressure sores, infected wounds, microcurrent as an adjunctive therapy, and the VeinoPlus device for venous ulcers.
Aetna's knee arthroplasty policy (CPB 0660) sets the criteria for total and unicompartmental knee replacement. Total knee arthroplasty is medically necessary for adult members with advanced joint disease, meaning pain and functional disability that interferes with ADLs, limited range of motion, crepitus or effusion, and moderate or severe knee osteoarthritis (Kellgren-Lawrence Grade 3 or 4) or another listed radiographic finding, after at least 12 weeks of documented non-surgical treatment (24 weeks for relative contraindications), with at least half of that treatment being formal in-person physical therapy in the past year. Revision of a total knee, unicompartmental arthroplasty and prophylactic tranexamic acid are covered under the listed criteria, while bicompartmental, customized, navigation-based and bacteriophage approaches are listed as experimental, investigational or unproven.
Aetna considers inflatable compression garments, non-elastic binders and pre-made medical grade (greater than 18 mm Hg) compression stockings medically necessary for listed conditions, including complications of chronic venous insufficiency, lymphedema, post-thrombotic syndrome, edema following surgery, fracture, burns or other trauma, and prevention of thrombosis in immobilized persons. Individually fitted (custom-made) medical grade stockings are medically necessary only for members who have tried and failed a 3-month trial of pre-made medical grade stockings, except for members with venous ulcers or lymphedema. Silver impregnated compression stockings are considered not medically necessary, and compression garments for the legs are considered experimental, investigational or unproven for listed indications such as Parkinson disease, delayed-onset muscle soreness and Long COVID autonomic dysfunction.
Aetna considers electrical stimulation for chronic ulcers medically necessary durable medical equipment for arterial ulcers, diabetic ulcers, Stage III or IV pressure ulcers, and venous stasis ulcers when it is used as adjunctive therapy after there are no measurable signs of healing for at least 30 days of conventional wound treatment. A course of treatment is not typically expected to exceed 60 minutes per day or a total of more than 4 weeks, and continued stimulation is not medically necessary if measurable signs of healing are not demonstrated within a 4-week treatment period. Electrical stimulation for chronic ulcers in the home setting is considered not medically appropriate, and it is considered experimental, investigational, or unproven for uses such as prevention of ulcers and pressure sores, treatment of infected wounds, and microcurrent as an adjunctive therapy to enhance chronic wound healing.
Aetna considers ligation, stripping, radiofrequency, endovenous laser and cyanoacrylate closure of the saphenous veins medically necessary when incompetence at the saphenofemoral or saphenopopliteal junction is documented by ultrasound performed within the past 6 months, with junctional reflux of 500 milliseconds or greater, a vein at least 4.5 mm in diameter below the junction, and the symptom criteria in the policy are met. Liquid or foam sclerotherapy (for example Varithena) is medically necessary for symptomatic saphenous veins, varicose tributaries, accessory veins and perforator veins that are 2.5 mm or greater in diameter when the criteria are met, and ambulatory phlebectomy is medically necessary as adjunctive treatment under the same size and symptom criteria. Treatment of veins less than 2.5 mm is considered cosmetic, endovenous ablation of varicose tributaries and accessory veins other than the accessory saphenous vein is considered experimental, investigational or unproven, and sclerotherapy for reflux at the saphenofemoral or saphenopopliteal junction is considered experimental.
Blue Cross Blue Shield Association policy 7.01.124 (BCBSMS policy A.7.01.124) states that treatment of great or small saphenous veins, accessory saphenous veins, varicose tributaries, and perforator veins may be considered medically necessary for symptomatic varicose veins or venous insufficiency when specific criteria are met, and that saphenous vein treatment not meeting those criteria is considered cosmetic and investigational. The policy history shows an update dated 09/22/2026 that changed the policy description, made minor changes to the statements for great or small saphenous veins, accessory saphenous veins, and perforator veins, and re-worded the concurrent treatment statements for accessory saphenous veins and symptomatic varicose tributaries for clarity. Telangiectasia treatment is considered cosmetic and investigational.
Several listed tissue-engineered skin substitutes may be considered medically necessary for specific uses, including breast reconstruction with listed acellular dermal matrix products, chronic noninfected full-thickness diabetic lower-extremity ulcers, chronic venous insufficiency ulcers that have not adequately responded to a 1-month period of conventional ulcer therapy, dystrophic epidermolysis bullosa, and second- and third-degree burns. All other uses of the listed products, and all other skin and soft tissue substitutes not listed, are considered investigational. Code reference updates dated 07/01/2026, 08/15/2026 and 10/01/2026 add CPT 1045T through 1049T, HCPCS C9353 and C9361, and HCPCS A2046, A2047, A2048 and A2050 as investigational codes.

This is historical material: an older Aetna policy (source dated 2025-12-02) for medicinal leech therapy and medical maggots (CPB 0556) that PolicyChanges found recently, and it states no effective date. It lists leech therapy for venous congestion and flap or replant salvage, and maggot debridement for certain non-healing wounds, as medically necessary, and it names many other uses, including knee osteoarthritis, low back pain and hemorrhoids, as experimental.
