Headaches: Nonsurgical Management (CPB 0462, reviewed 2026-07-23)
Aetna covers intravenous dihydroergotamine (DHE) for status migrainosus, for other intractable severe migraine attacks unresponsive to analgesics and triptans, and for cluster headache attacks that do not respond to oxygen or triptans, when given in the emergency room, hospital or urgent care setting, and covers it for medication overuse headache in the inpatient setting. Aetna also covers intramuscular ketorolac for short-term management of acute migraine (5 days or less), intramuscular and intravenous steroids and intravenous magnesium for acute migraines, caffeine citrate infusion for post lumbar puncture headache when the member is unable to take caffeine orally, and eptinezumab-jjmr (Vyepti) for preventive migraine treatment in an adult when the criteria are met. Aetna considers cranio-sacral therapy, intra-oral splints for tension-type headaches, IV DHE for other headache types, and a list of other nonsurgical migraine and cluster headache interventions experimental, investigational or unproven.
