Diagnosis and Treatment of Sacroiliac Joint Pain
Sacroiliac joint arthrography under fluoroscopy with injection for diagnostic purposes, with or without anticipation of lumbar fusion, is considered medically necessary, and up to two injections per calendar year are allowed. Minimally invasive fixation or fusion of the sacroiliac joint using a titanium triangular implant may be considered medically necessary only when all listed criteria are met, including at least 6 months of failed intensive nonoperative treatment and at least a 75% pain reduction on two separate image-guided injections. Fixation or fusion for other conditions or with other devices, and radiofrequency denervation of the sacroiliac joint, are considered investigational.
