Standing Devices and Gait Trainers 1.01.46
Excellus BlueCross BlueShield policy 1.01.46 considers a standing device medically appropriate when documentation shows it is necessary for the patient to be independent in eating, personal hygiene, toileting, dressing, or transfer at home and the patient cannot accomplish those activities with current durable medical equipment. A standing device for decubitus ulcer management requires that off-loading cannot be accomplished by other means and a completed one-month trial showing meaningful improvement, and a gait trainer requires documented mobility limitation, the potential for ambulation, and inability to perform the listed activities with current equipment. The policy also sets repair and replacement criteria, and the version effective February 19, 2026 is an annual update with no changes to policy intent.
