Orthognathic Surgery (Overbite/Underbite)
Orthognathic surgery is considered medically necessary when a maxillary or mandibular skeletal deformity causes masticatory malocclusion and meets one of the listed measurement criteria, or when obstructive sleep apnea has been treated unsuccessfully with CPAP and all listed criteria are met. Orthognathic surgery performed for malocclusion when these criteria are not met is considered not medically necessary. Surgery performed primarily for cosmetic purposes, including procedures to reshape or enhance the chin, is not eligible for coverage.
